A COVID-19 vaccination clinic will be at Perkins High School for students 16 years old and older, their families, and school district staff on Tuesday, May 4. The clinic will be run by the Erie County Health Department and Erie County Community Health Center staff.

The clinic will be from 4 p.m. to 7 p.m. in Room 301 of the Perkins Schools Administrative Service Center at the high school building. The Pfizer vaccine will be given at the clinic. Everyone who receives a vaccine on May 4 must return on Tuesday, May 25, to receive the second dose of the vaccine.

Pre-registration and an appointment for the vaccine are required by calling the Health Department’s central scheduling center at (419) 626-5623, Ext. 5174. Due to the vaccine management requirements and the need to minimize vaccine waste, appointments will be booked in groups of six individuals.

On the day of the clinic, individuals should arrive at their scheduled appointment time, and enter through the south entrance of the Perkins High School building.

Face masks are required. A parent or legal guardian must accompany and be present for the vaccination and sign the consent form for anyone under 18 years old. The parent or legal guardian must have photo identification and insurance card, if insured.

There is no cost for the vaccine. The Erie County Health Department will submit a vaccine administration charge to an individual’s insurance, and there is no out-of-pocket expense for any individual receiving the vaccine. Anyone who is uninsured may receive the vaccine at no cost.

Additional appointment dates and times are available at the Erie County Health Department/Erie County Community Health Center if this date and time is not convenient.
