Background Checks

Perkins Schools performs background checks in-house for employees and new staff members, coaches and advisors, and parents, volunteers, and chaperones. To schedule a time, contact Dawn Sullivan via email at or phone 419-621-2061.

Parents/Volunteers/Chaperones: In keeping with state background check laws and to ensure the safety students, all parents and community members who wish to volunteer at our buildings and/or accompany a child on a field trip must have a current BCI background check less than 5 years old on file at the district office. We appreciate your cooperation, and thank you for volunteering on behalf of our students.

• Athletic (Paid Supplemental) and Athletic Volunteers are required to have background checks for Pupil Activity Permit 
• Volunteers and Chaperones also need FBI check if not an Ohio resident for previous five years
